Check Request Form

A church member would use this form when a vendor needs to be paid for services that were provided to the church or when the church member needs to be reimbursed for church expenses that they paid for out of their own pocket.  Supporting invoices(s) and/or receipt(s) must be submitted with this form.  Please make sure that the appropriate department head has signed the check request before submitting all the paperwork to the Church Treasurer.

 

Financial Assistance Request Form

This form is used by the Community Services Director as part of the process to request financial assistance from the Church to a community member or church member that has a financial need that they cannot meet themselves.  Please remember that financial assistance is never given directly to the requesting individual and that limits apply.  Submit the form to the Church Treasurer for approval by the Finance Committee.

 

Project Proposal Form

A church member or department head would use this form when they want to do a project that requires Church Board and/or Church Body approval.  The most common use of this form would be when a department head wants to spend more than their annual budget.  This form should be brought to the Church Board or Church Business Meeting where the project is presented and given to the Church Treasurer.

 

Member Facility Use Agreement

A church member or department head would use this form when they want to use the church or BLC for an event.  Note that if an outside organization wants to use the church or BLC for an event a separate application is used that also requires Arizona Conference approval.  Submit the form to the Church Treasurer for approval by the Facilities Committee and then the Church Board.

 

Designated Gift Form

A church member would use this form when they want to donate significant funds to a very specific church project.  Submit the form to the Church Treasurer for possible approval by the Church Board. 

 

Offering Counting Receipt Form

Deacons, treasury staff or other departmental helpers would use this form when they count the funds from an offering collection.  For each offering collection two receipts need to be filled out.  One of the receipts would be kept with the offering funds and given to the Church Treasurer and the second receipt would be stored in a manila envelope in the church office.
